This program serves adults who have a primary diagnosis of epilepsy and are capable of living independently with support services are provided 24 hour emergency assistance and monitoring services by People Inc. Clients are taught independent living skills and can receive individual counseling and support services necessary to attain their goals.

Provides services to elderly adults requiring assistance with independent living. Services provided are assistance with personal care, weekly housekeeping, and case management, two nutritionally balanced congregate meals daily, emergency response-24 hours a day, assistance with medications, bathing assistance, planned recreational activities and on-call nursing. Maximum Client Capacity: 40

This 3-5 story midrise with 239 units; provides programs for elderly adults & adults with disabilities who need assistance – personal care, case management, two nutritionally balanced congregate meals daily, weekly housekeeping assistance, emergency response 7:00 a.m. to 11:00 p.m., assistance with medications, weekly bathing assistance, planned recreational activities and on-call nursing. Maximum client capacity: 30 (Thirty)

(Assisted Living Program, Volunteers of America); 1815 Central Avenue NE, Minneapolis, MN 55418

This 25 story, 333 unit building provides services for elderly adults, as well as adults with disabilities. Services provided are assistance with personal care, case management, two nutritionally balanced congregate meals daily, weekly housekeeping, emergency response 7:00 a.m. to 11:00 p.m.; assistance with medications, weekly bathing assistance, planned recreational activities and on-call nursing. Maximum client capacity: 40 (Forty)

(Senior Living with Services, Cassia); 2533 1st Avenue South, Minneapolis, MN 55404

This seven story, 42 unit building, provides services for elderly adults. Services include: onsite nursing , on call nursing after hours, assistance with personal care, case management, three nutritionally balanced congregate meals, weekly housekeeping, weekly laundry, 24 hour emergency response, assistance with medications and planned recreational activities. Maximum Client Capacity: 42

(Home Care Services and Memory Care Program, Cassia); 901 4thAvenue North, Minneapolis, MN 55405

This 4 story 48 unit ‘smoke free’ development provides a more supportive environment with a culture of person-centered care, services tailored to residents’ unique needs and preferences with apartments that are designed specifically for individuals experiencing cognitive changes. Services include: Assessment by Registered Nurse, diabetic care options including sliding scale insulin, three meals per day, weekly light housekeeping, weekly laundry services, monthly vitals, emergency response system, social and life enrichment programs and home health services 24 hours a day 7 days a week. Memory Care includes all of the above as well as secured areas, cognitive screening performed by Occupational Therapist to determine the proper level of care and staff specifically trained in the care of dementia residents. This development is specifically designed for adults over 55 who need enhanced assisted living and memory care services.
