How the Data Practices Act Applies to MPHA and How to File a Request

The Data Practices Act (Minnesota Statutes, Chapter 13) is Minnesota’s law covering data privacy and data sharing by public entities. It is similar to the Freedom of Information Act, or FOIA, at the federal level. As a state-chartered entity, the DPA (rather than FOIA) applies to MPHA. If you seek to request information under the Minnesota DPA, please use the following forms. (Note: If you are a current or former employee seeking information about yourself, do not use these forms. A DPA request is likely unnecessary. Please call MPHA Human Resources at (612) 342-1442, or email HR@mplspha.org.)

These forms are fillable PDFs, which require software such as Adobe Reader. Depending upon your browser, you might need to download them to your computer in order to fill them out and save them. You may also print them and fill them out by hand.

Once complete, please save your changes and email your form to DataPractices@mplspha.org.

Email is our preferred method to receive the forms. However, you may also drop them off in-person during business hours (to assure best processing, please state that they are for our Legal Department) or mail to “Data Practices Requests, Attn: MPHA Legal Department, 1001 Washington Ave. N., Minneapolis, MN 55401.” Note that any DPA timelines do not begin until we receive the request.
