The Data Practices Act (Minnesota Statutes, Chapter 13) is Minnesota’s law covering data privacy and data sharing by public entities. It is similar to the Freedom of Information Act, or FOIA, at the federal level. As a state-chartered entity, the DPA (rather than FOIA) applies to MPHA.

Depending upon your browser, you might need to download them to your computer in order to fill them out and save them. You may also print them and fill them out by hand.
